This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 36C25018Q0100 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 & 2005-95. The associated NAICS code 339112 is Surgical and Medical Instrument Manufacturing and small business size standard is 1,000 employees. This is a 100% total Service Disabled Veteran Owed Small Business (SDVOSB) set-aside procurement.
The Louis Stokes Cleveland VAMC is looking for a brand name or equal to the Mass Medical Endoscope Cabinet and associated components, see table below per FAR part 11.104. Vendor must provide product listed or can propose an equivalent product if it matches the specifications provided. Specifications must be submitted for all products if an or-equal product is being submitted.

Specification for Scope Cabinet
Cabinets must be constructed of stainless steel.
Must be assembled in a European nation (Switzerland, Belgium, Germany, France, UK) or the US.B If offered from other countries not listed, they will not be considered.
Must include specific indications for use (IFU) that will describe the complete cleaning and operating parameters
Must be turnkey installed by the vendor (also to include disassembly and removal of existing cabinets)

Must meet or exceed recommended guidelines for scope storage as described in AORN, SGNA, ASGE, APIC, AAMI ST 91, Joint Commission and VHA Directive 116(2)
Endoscope must be able to be hung vertically with the distal tip hanging freely in a well-ventilated, dust-free area, following the manufacturer s written IFU for storage.
Must prevent coiling or kinking of the flexible endoscope
Cabinet must be closed that allows air circulation around the endoscopes
Cabinet internal surfaces must be composed of cleanable material and must be able to be cleaned and disinfected on a weekly schedule with a hospital-approved disinfectant.
Height of the cabinets must allow endoscopes to hang without touching the bottom of the cabinet
Cabinet must provide sufficient space for storage of 9 flexible endoscopes without touching.
Must offer HEPA-filtered air to provide positive pressure in the cabinet and reduce the incidence of possible bacterial contamination.

EVALUATION CRITERIA/ SUBMISSION OF OFFER
Evaluation: Using the simplified procedures under FAR 13, Offers will be evaluated using: price, ability to meet the specifications (if not providing the brand name), and delivery.
