Seeking proposals on services for the provision of the following services to elderly individuals for the period July 1, 2008, to June 30, 2009:
# Case Management
# Congregate Meals
# Home-Delivered Meals (Federal)
# Home-Delivered Meals (State)
# Legal Assistance
# Nutrition Education
# Nutrition Transportation
# Outreach
# Public Education
# Transportation
Application packets will be available at our office or by calling.
Questions on the contents of the application packet should be directed to the Office of Aging contact person as noted on the Request for Proposal: Service Specifications.
All proposals must be received by 4:30 p.m. on March 7, 2008 at the County Office of Aging. Proposals will not be accepted after the deadline. All proposals must be complete and submitted in accordance with the Request for Proposal Guidelines. Inquiries regarding this Request for Proposals should be directed to Pauline Fukunaga.
