A Service Partnership is an array of services and providers organized to provide a comprehensive services that are well integrated to meet the individual service needs of children and family with multiple, indicated risks. These services are
not to be found within any single agency and require a collaborative partnership of varied health and social service agencies in a shared governance structure with community involvement, buy-in and ownership. Successful Service
Partnerships will utilize gMemorandums of Understandingh to clearly delineate how member agencies will work together to implement gsystem of care principlesh. The Service Partnership collaborators will work holistically with families and community to achieve desired, measurable outcomes in the chosen program/focus area. The service system design must include gbest practiceh models for addressing the needs of children and their families.. Each Service Partnership will utilize a process of community needs assessment and asset
mapping to determine which program/focus area is best indicated for the targeted geographic area or at-risk population. The four program/focus areas that are priorities of The Children's Trust are as follows:
maternal, infant, and child (ages 0-5) health and development needs;
safety, injury and trauma prevention; and/or the prevention of family violence, child abuse or neglect (0-18);
prevention of risky behaviors among adolescents (ages 13-18) which can lead to delinquency, gang involvement, school dropout, pregnancy, sexually transmitted diseases, tobacco, alcohol and substance use/abuse, underemployment or unemployment;
the special needs of children with disabilities and their families (0-18)
Respondents wishing to address other program/focus areas may do so but must convincingly justify the need, and distinguish the proposed area from the four program/focus areas listed above.
