The 2012 Robert Wood Johnson Foundation Community Health Leaders awards program will recognize individuals working to address health challenges in their own communities. Nominees may be someone doing exceptional work to improve local health or access to healthcare, or someone who has solved a daunting community health problem.
The Robert Wood Johnson Foundation selects ten unsung heroes each year to receive the award, which includes national recognition, opportunities to network and collaborate with fellow health leaders around the country, and $125,000 in support of the honoree's work. The winners receive tools and knowledge to help them continue their efforts to improve health and healthcare where they live.
Selected leaders come from diverse professional backgrounds and regions of the country. Recent award winners are providing compassionate care to dementia patients; supporting lesbian, gay, bisexual, and transgender youth; providing free health care to homeless women; and developing support services for brain injury survivors.
To be considered for a CHL award, a nominee must have contributed significantly to improving the health of underserved communities by increasing access to health services or quality; have positively affected a significant number of people in the targeted community; have been involved with the initiative for which he/she is nominated for no less than three years and no more than ten years; have a record of accomplishment associated with the community health issue for which he/she is nominated; be a relatively unsung leader who has not received significant national recognition or awards for his/her work; be a citizen or permanent resident of the U.S. or its territories; and be affiliated with a public or nonprofit 501(c)(3) organization.
Nominations should come only from people who know their candidates well, who are personally inspired by them, and who will actually write the nomination. Nominations from development and public relations professionals, professional grant writers, and former recipients of the award will not be accepted. A nominee may not nominate him/herself or be a relative of the nominator.
