Medical Oxygen and Nitrous Oxide
The City is accepting competitive sealed bids for an Annual Contract for Medical Oxygen and Nitrous Oxide. The term of this contract will be through September 30, 2010 with the option to extend up to two (2) additional twelve (12) month periods, subject to the approval of the supplier and the City.
*It is the intent of the City of Lewisville to contract with one (1) vendor for all materials, labor and equipment necessary to provide medical oxygen to City of Lewisville Fire/EMS locations.
*All pricing must include shipping, handling, inside delivery, surcharges and set-up FOB to the Lewisville Fire Departments specified in the bid.
*Quantities are estimates and the City reserves the right to change quantities based on the City's needs.
*Upon notification of award vendor must provide a certificate of insurance in compliance with the attached insurance requirements. The certificate must be approved by the City and coverage must be maintained throughout the term of the contract. Work may not commence until these requirements are met.

The closing date of this bid has been extended to January 5, 2010 pending further review of the specifications.
