HEALTH CARE PROVIDERS TO FURNISH SERVICES FOR PUBLIC HEALTH NURSING
The owner is seeking services for a three (3) year contract from a healthcare provider to provide skilled qualified and licensed personnel to perform all supervision and services necessary for Public Health Nursing Services.
The owner is specifically seeking services for the municipalities of Somerville, Raritan, Manville and the Township of Franklin. The owner is responsible for the provision of public health services according to the Recognized Public Health Activities and Minimum Standards of Performance for Local Boards of Health in New Jersey (revised) as set forth at N.J.A.C. Title 8 Chapter 51. The services being sought through this RFP include Child Health Conferences, Maternal and Child Health visits (includes lead screening and home birth verification), Health Maintenance Programs, Chronic Disease Detection Programs, Audit of School Immunization Records, Adult Immunizations and Communicable Disease Control.
