HEALTH CARE PROVIDERS TO FURNISH SERVICES FOR CHILD HEALTH CLINIC PHYSICIAN
1. The owner is seeking services for a three (3) year contract from a healthcare provider to provide skilled qualified and licensed personnel to perform all supervision and services necessary for Child Health Clinic Physician.
2. The owner is specifically seeking services of a NJ Licensed Physician for the municipalities of Somerville, Raritan, Manville and the Township of Franklin. The owner is responsible for the provision of public health services according to the Recognized Public Health Activities and Minimum Standards of Performance for Local Boards of Health in New Jersey (revised) as set forth at N.J.A.C. Title 8 Chapter 51. The service being sought through this RFP is for general health assessment of pediatric patients seen at Child Health Clinics. Respondents may offer proposals for either or both of the clinic locations identified below.
3. Clinic related services will take place at two locations: the Franklin Township Clinic at 935 Hamilton Street and at a combined Manville Borough, Raritan Borough, and Somerville Borough clinic (Combined Clinic) to be held at a site to be determined by the owner. Respondents may bid on either or both clinic locations.
