Community Wellness Grants and Harkin Wellness Grants for Local Health Improvement
The purpose of this RFA is to provide guidance for current contractors to submit continuation applications for state fiscal year 2011, the second (2nd) and final year of the project period established by the RFP #58809022, RFP #58810011 and RFP #58810020. Community public health services covered by this application include coordinated programs at the community level to encourage healthier lifestyles, promote wellness and prevent the health and financial consequences of chronic disease that are often lifestyle related. This RFA serves to:
1. Maintain and advance integration of public health activities,
2. Maintain and advance delivery of essential services and core public health functions,
3. Build stronger relationships with community partners.
