Chronic Disease Billing & Claim Processing
The purpose of this Request for Proposal (RFP) is to solicit proposals that will enable the State to select the most qualified applicant to provide claim processing and claim reimbursement based on Medicare Part B Participating Provider rates for program-approved Current Procedural Terminology (CPT) codes, and activity reports for chronic disease direct service programs. The Department is responsible for funding claim processing and claim reimbursement services to approved healthcare providers and facilities for screening and diagnostic services for program participants.
