The Boston Public Health Commission REACH Center of Excellence in the Elimination of Disparities is pleased to release this Request for Applications (RFA) for New England public health agencies and coalitions to participate in a multi-state learning collaborative designed to build regional capacity to address root causes of health disparities and the social determinants of health and improve health outcomes in Black/African American communities. In October 2007, 18 Centers of Excellence in the Elimination of Disparities (CEEDs) were established across the US by the Centers for Disease Control and Prevention and its Racial and Ethnic Approaches to Community Health Across the U.S. Cooperative Agreement to serve as resources for mentoring communities (aka Legacy Projects) in processes of community mobilization, community-based participatory research, program development, and evaluation. The New England CEED at the Boston Public Health Commission is one of these centers. It will facilitate this 3-year collaborative project and provide training, technical assistance and small infrastructure-building grants to help develop, implement, and coordinate a comprehensive strategy to address health inequity and eliminate racial and ethnic disparities in health. Components of such a strategy may include: addressing institutional racism, engaging and mobilizing the community, collecting and using data to target intervention opportunities and track progress, building the capacity of community institutions, conducting community education, advocating for health systems and policy changes and making neighborhood investments to support community wellness.
