1. The Contractor shall provide the following products and services for Health and Human Services, Indian Health Service (IHS), Uintah & Ouray (U&O IHS Service Unit. The physical address is 6822 E. 1000 S., Ft. Duchesne, UT 84026.

2. The following equipment and supplies will be included as part of the television package in accordance with requirement commensurate with the Medical Facilities need: Two (2) DVR receivers, cables to be run for the service inside and outside the building. Current equipment in place and being utilized is as follows:


a. Two (2) DISH Network ViP 722K DVR Receivers
b. One (1) DISH HD Plus exterior mounted satellite dish


3. Initial Services. The contractor shall provide services at the IHS Wellness Center in Ft. Duchesne, UT.


4. Equipment Check In. The task associated with tracking and accountability (service) shall be coordinated upon receipt of order. The following personnel shall be notified during this phase: U&O Property Office and personnel utilizing the equipment in the U&O IHS Wellness Center building. The points of contact are as follows:

6. EQUIPMENT INSTALLATION. The contractor shall provide a transition plan for the installation of all interior equipment (receivers) and exterior equipment (satellite), if required.


7. Statement of Need: As part of improving patient care, enhancing the patient experience (customer service), and for direct patient care services, TV service is needed within the gym and education room at the U&O IHS Wellness Center. The TV helps engage and entertain patients, which helps them exercise longer, thus improving patient's health and fitness. The music helps people work out at a higher intensity, thus improving their cardiovascular fitness. Television service is an industry standard and is common in commercial fitness centers and is requisite for a wellness center facility.


8. Existing equipment in the Wellness Center that need service are as follows:


a. One (1) television in the education room
b. Three (3) televisions in the fitness room. One TV (mounted on the side of the room) is dedicated to play satellite radio while the other two TV's are in the front viewing area and are set to both play different TV shows simultaneously.
