Proficiency Testing Samples
This Sources Sought Synopsis (SSS) is in support of Market Research being conducted by the Phoenix Indian Health Services to identify potential contractors to compete, to determine if this effort can be competitive, or a Small Business Set-Aside is possible. This is not a Request for Proposal (or solicitation) or an indication that a contractual commitment will exist for this requirement. The government does not intend to award a contract on the basis of this market research not until the end of this year.
The Phoenix Indian Health Services located in Phoenix, Arizona is requesting information, pursuant to FAR PART 10, in support of Market Research to identify potential sources to provide provide continued operation for Whiteriver PHS Service Unit at 400 W. Apache Drive, Whiteriver AZ and Cibecue Health Center at Cibeque, AZ 85941 for proficiency testing in order to maintain accreditation with Joint Commission (JC) and Center for Medicare & Medicaid Services (CMS). As a moderately complex laboratory, the Whiteriver Service Unit and Cibecue Health Center require enrollment in a proficiency-testing program according to CLIA '88 regulations. This project consists of maintaining accreditation by utilizing a CMS approved proficiency program.
The proposed North American Industry Classification Systems (NAICS) Code is 611310. This project consists of maintaining accreditation by utilizing a CMS approved proficiency program, which has a corresponding Size Standard of 1,500 employees. The Government is interested in all businesses to include, Other than Small Businesses, Small Businesses, Small Disadvantaged Businesses 8(a)s, Service-Disabled Veteran-Owned Small Businesses, Hubzone Businesses, and Women-Owned Small Businesses. The government requests that interested parties respond to this notice if applicable and identify your socio-economic status to the identified NAICS code.
The contractor shall be capable to perform in accordance with the attached statement of work.

2. Proficiency Testing Samples for Cibecue 5-years
