Orthotist Services Tomah VA Medical Center
The Great Lakes Acquisition Center (GLAC), on behalf of the Tomah Veterans Affairs Medical Center (VAMC) located at 500 East Veterans Street, Tomah, WI, 54660, is soliciting quotes for a contractor to provide one American Board Certified (ABC) Orthotist for the Orthotics Clinic at the Tomah VA Medical Center (VAMC). All work performed under this contract will be completed at the VA.
Typically, these clinics are held on the second Thursday of the month from 8:00 a.m. to 12:00 p.m. and from 12:30 p.m. - 4:30 p.m. as well as on the fourth Thursday of the month from 12:30 p.m. - 4:30 p.m. These clinics are held to measure, fit and adjust prosthetics. The Orthotist performing the fittings needs to have the capability to fabricate required prosthetics, but they will be billed separately outside of this contract. The anticipated period of performance is March 25, 2019 through March 24, 2020, with four one-year options through March 24, 2024.
The NAICS for this requirement is 621399 Offices of All Other Miscellaneous Health Practitioners, which has a small business size standard of $7.5 million. These services have been determined to be non-personal services. Quotes, including options, will be evaluated for fair and reasonable pricing, technical capability to perform services, SDVOSB/VOSB Status as verified in VIP, and a satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. The resultant contract type will be a fixed price contract.
