This is a combined synopsis/solicitation for commercial items prepared in accordance with format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The solicitation number for this acquisition is IHS1374390. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures.

The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02, effective May 5, 2019.

The North American Industry Classification System (NAICS) Code for this acquisition is 611710 - Educational Support Services with a business size standard of $15.0 Million.

The Navajo Area Indian Health Service, Northern Navajo Medical Center has a requirement for Contractor shall provide consultation services in support of wellness activities. The period of performance is estimated for 11 months. See attached statement of work for additional information.

The Contract Line Items (CLINs) and pricing structure are as shown below.

CLIN 1: Contractor shall plan, develop and implement Teen Parent Educator.
CLIN 2: Contractor shall provide direct consultation in adaptation of health services for Impact Concussion equipment training for Central Consolidated School District.
CLIN 3: Contractor shall provide wellness consultation and access to exercise equipment and swimming pool
CLIN 4: Contractor shall provide support in community based HPDP promotional events related to wellness activities and other HPDP.
CLIN 5: Contractor shall provide Mental Health Wellness Initiatives for the students, parent, and community of Shiprock Service Unit.
CLIN 6: Contractor shall develop school and community early response to heart disease programs that shall include Automated External Defibrillators (AED) to staff and community training.
CLIN 7: Contractor shall provide Crisis Prevention Training for the HPDP staff at Shiprock Service Unit.
CLIN 8: Contractor shall provide Health training and supplies for student, parent and community within the Shiprock Service Unit.

The provision at 52.212-1, Instructions to Offerors-Commercial (OCT 2018), applies to this acquisition.

52.212-2, Evaluation-Commercial Items, the award will be made to the lowest priced technically acceptable offeror.

Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (OCT 2018), with its offer.

FAR 52.212-4, Contract Terms and Conditions-Commercial Items (OCT 2018), applies to this acquisition.

52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (May 2019), applies to this acquisition.
