Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation document will not be issued.
This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01, effective 22 January 2019.
The associated North American Industrial Classification System (NAICS) code for this procurement is 621999, with a small business size standard of $15.0 million.
The Department of Veterans Affairs Healthcare Systems at VA North Texas Health Care System (Dallas) requires a contractor to provide Extracorporeal Shock Wave Lithotripsy (ESWL) Services with an On-Site Technician. This is a Firm Fixed-Price Contract.
Period of performance shall be for one (1) Base Year with the Government having the option to exercise four (4) one-year option periods.
General Timeline:
i. Proposals are due Friday, 5 April 2019 at 1200 PM MST
ii. Contractor performance shall begin on Wednesday, 1 May 2019.
