THIS REQUEST FOR INFORMATION IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION.

This is a Request for Information (RFI), this is not a Request for Quotes (RFQ) or Request for Proposals (RFP). No formal solicitation document exists at this present time. This is not a solicitation announcement. The purpose of this Sources Sought Notice is for market research to gain knowledge of potential vendors qualified under NAICS code search 334510 that can provide One Holter Monitor with 16 Recorders. NAICS code 334510 includes corresponding index entries such as Electromedical and Electrotherapeutic Apparatus Manufacturing with a size standard of 1,250 employees.

Responses to this Sources Sought Notice will be utilized by the Government to make appropriate acquisition decisions. After review of the responses to this RFI, a solicitation announcement may or may not be published In GSAebuy.

This Sources Sought Notice is to identify potential vendors with the resources and capabilities to provide the aforementioned product to the following Veterans Affairs Medical Center (VAMC):

REQUESTED INFORMATION

If you are interested, and can provide the required supplies/services, please provide the requested information as indicated below. Responses to this Sources Sought Notice shall also include: company name, address, point of contact, and size of business:

Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number.

Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) with an active registration in VETBIZ, HUBZone, Woman Owned Small Business (WOSB), Small Business, Large Business, etc.)

All Capability Statements/Responses to this Sources Sought Notice shall be submitted via email only to David Valenciabonilla, email: David.Valenciabonilla@va.gov. Telephone, fax, or any other form of communication enquiries will not be reviewed or accepted. Responses to this Sources Sought Notice must be received no later than July 23, 2019 at 8:00 a.m. (Eastern Standard Time). This Sources Sought Notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued, it may or may not be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this Sources Sought Notice.
