PERFORMANCE WORK STATEMENT
FITNESS EQUIPMENT MAINTENANCE
NELLIS AFB, NV
20 MARCH 2019


Sect ion 1: Description of Service:


1.1. General Requirements: The contractor shall provide all personnel, equipment, tools, material, supervision, and other items/services necessary to perform weekly preventative maintenance and servicing of all equipment at the Nell is AFB Warrior Fitness Center, as specified within this Performance Work Statement (PWS).


1.2 Initial Acceptance Inspection: The contractor will do an initial acceptance inspection of equipment NLT 30 Sep 2019. The contractor will notify the designated authorized government representative in writing of any delays beyond the contractors control and request for an extension of the initial inspection. Failure to notify the designated authorized government representative before the specified initial acceptance inspection completion date shall constitute acceptance by the contractor of all equipment listed in the appendices of this PWS.


1.2.1 Initial Acceptance Inspection Equipment Serviceability: Equipment is considered serviceable and in good working condition if it is performing all its intended functions and there are no missing or broken parts. Equipment shall not be rejected on the basis that it needs cleaning or adjustments.


1.2.2. Initial Acceptance Inspection Completion: Upon completion of the initial acceptance inspection, the contractor must notify the designated authorized government representative, in writing, of any fitness equipment that is unserviceable and/or needing repair. The contractor must state the reasons for the rejection and the repairs needed to bring the fitness equipment to an acceptable working condition. The contractor is not responsible for any rejected fitness equipment until such time as accepted by the contractor.


1.2.3. Added Fitness Equipment: Fitness Equipment added to the PWS after the start of the period of performance will be subject to the same initial acceptance inspection, by the contractor, under the same terms and conditions as stated in this PWS.



1.3 Preventative Maintenance and Servicing: Contractor shall, in accordance with the equipment manufacturer's most current technical and maintenance literature and current industry practices perform monthly preventative maintenance and servicing of all fitness equipment as specified in the appendices of this PWS. All equipment shall be inspected and/or serviced a minimum of one (1) time per month. Preventative maintenance includes, but is not limited to: remove, install, replace broken and/or missing parts; lubricate bearings, rods, chains, pulleys, cams and seat adjustments; apply appropriate chemicals on metal parts to prevent rust/corrosion; clean, disinfect and detail all frames, housings and upholstery; clean, test and inspect all electrical connections and components. Suggestions for improvement and efficiency of fitness equipment operation should be offered by the contractor's service personnel to government personnel at the time services are made.



1.4 Government Responsibilities:



1.4.1 Authorized Government Personnel: An Authorized Government Personnel Listing, in coordination with the Contracting Officer, will be provided, in writing, to the contractor within ten (10) working days after receipt of the written purchase award. The listing will identify government personnel authorized to inspect and accept the contractor's work. Authorized government personnel may represent the government only to the extent authorized in writing. Only the Contracting Officer can obligate the government. The Government will not be responsible for work calls placed by individuals other than those authorized, in writing, by the Contracting Office.
1.4.2. Utilities: Water and electricity are available for use by the contractor at the Fitness Center. Contractor may utilize water and electricity in quantities necessary to perform the work, at no cost to the Contractor. Contractor will be responsible for all temporary connections necessary for operations. Temporary connections will be removed prior to final acceptance of work.


1.4.3 Work Area: Government will provide adequate work space for all preventative maintenance and servicing.


1.4.4 Parts: The contractor furnishes all parts costing $25.00 or less at no cost to the government. Only the original manufacturer's standard parts, or verifiable equal, will be used in performing preventative maintenance and servicing. Parts exceeding $25.00 will be independently purchased/furnished by the government. All replacement parts become property of the government.


Contractor Responsibilities:


1.5.1 Waste and Debris: Contractor is responsible for all waste products generated. At no time will the contractor dispose of hazardous material or hazardous waste on base. Waste must be properly contained at all times and removed from Nellis AFB and disposed of properly. Access to a base dumpster may be made available for certain non-hazardous debris and rubbish by an authorized government representative. Contractor must clean up the work area upon completion.


1.5.2 Warranty of Work: Contractor shall warranty any and all workmanship in accordance with the equipment manufacturer's most current technical and maintenance literature and current industry practice or for a minimum of one (1) year if no such warranty exists. Contractor shall, at no cost to the government, promptly furnish and install replacement parts and correct defects in workmanship upon receipt of notice from the authorized government representative concerning failure of any part of the system during the warranty period due to contractor neglect.


1.5.3 Responsibility of Government Property: Contractor assumes full responsibility and shall indemnify the government for any and all loss/damage resulting, in whole or in part, from acts of neglect or omissions by the contractor, subcontractor, or any employee, agent, or representative of the contractor or subcontractor, to any government property while in the contractor's custody and care for storage, repairs, or services to be performed under terms of this PWS.


1.5.4 Reporting: Contractor is required to report to an authorized government representative at the Warrior Fitness Center prior at the start of any work and at the completion of all work. Receipt of work must be provided for, but not limited to, all preventative maintenance and servicing to verify the performance of work. The receipt must include, but is not limited to, the name of the person performing the maintenance or service, fitness equipment inspected, date and signature of the machine operator or equipment custodian and the contract number.



1.5.5 Receipt: Contractor will furnish all receipts of work and hand receipt for approval by an authorized government representative.

4.1 Warrior Fitness Center Hours of Operation: All work shall be accomplished during normal fitness center hours of operation as follows: 8 A.M to 5:00 P.M., Monday through Friday, excluding federal holidays.
4.2 Base Security/Pass: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Government installation, shall abide by all security regulations of the installation. The contractor shall obtain from the Government an AF Form 75, Personal Identification Card, for each employee working at Nellis AFB, and AF Form 533, Vehicle Registration Certificate for personal vehicles requiring access to Nellis AFB. When any employee no longer works for the contractor at the installation, the contract manager shall return any Government furnished identification to the Government. These passes are at no cost to the contractor. Use of photographic equipment is forbidden without express consent of the Contracting Officer.


4.3. Safety: Contractor is responsible for complying with all Occupational Health and Safety Administration (OSHA) guidelines.


4.4. Location: Work site is located at Bldg 1705, Warrior Fitness Center, Nellis AFB, NV.


4.5. Government Representatives: The contractor will receive designation of the authorized government representatives, contracting officer and contract administrator with contract award or in accordance with this PWS.


4.6 Contractor's Failure to Perform: If contractor fails to perform required services within the specific limitations, the government may:


4.6.1. Accept the services at a later date.


4.6.2. Refuse to accept the service at a later date in which case the government will bear no cost and make no payment for the missed service.


4.6.3. Procure the missed service, upon such terms and conditions as the Contracting Office deems appropriate, from an alternate, responsible firm and the contractor shall be liable to the government for any excess costs incurred and to continue the performance as required thereafter.


4.7 Invoices and Payments: The contractor's must be registered and invoice the Warrior Fitness Center Director or designated representative under Wide Area Work Flow.
