This Notice seeks to locate qualified Nursing Home facilities to provide a full range of nursing home care to Veterans enrolled at the Tennessee Valley Healthcare System (TVHS) and James H. Quillen VA Healthcare System (Mt. Home). TVHS/ Mt. Home has a need for Medicare approved additional Nursing Home facilities in certain locations, and is specifically seeking Nursing Home facilities in the following locations: Davidson County, TN; Hamilton County, TN; Montgomery County, TN; and Williamson County, TN. Interested Long-Term Nursing Home facilities are requested to submit an application at their earliest opportunity.
Background: The Tennessee Valley Healthcare System (TVHS), Nashville Campus located at 1310 24th Ave S, Nashville, TN 37212, Alvin C. York Campus Located at 3400 Lebanon Pike, Murfreesboro, TN 37129 and the James H. Quillen VAMC, Corner of Lamont Street and Veterans Way, Mountain Home, TN 37684 has a requirement for Community Nursing Home services for Veterans in accordance with the attached Statement of Work (SOW). Nursing home facilities are need in the following Tennessee, Kentucky, Virginia, and Georgia Counties: Calloway, Stewart, Benton, Perry, Wayne, Huston, Humphreys, Hickman, Lewis, Lawrence, Montgomery, Dickson, Cheatham, Williamson, Murray, Giles, Robertson, Davidson, Rutherford, Marshall, Bedford, Moore, Lincoln, Sumner, Wilson, Macon, Trousdale, Smith, Jackson, Clay, Pickett, Overton, Fentress, Scott, Morgan, Dekalb, Cannon, Coffee, Franklin, Putnam, White, Warren, Grundy, Marion, Cumberland, Van Burren, Bledsoe, Sequatchie, Rhea, Meigs, Hamilton, Bradley, Polk, Monroe, McMinn, Blount, Roane, Sevier, Knox, Anderson, Campbell, Claiborne, Union, Jefferson, Cocke, Hamblen, Grainger, Hancock, Hawkins, Unicol, Carter, Johnson, Washington, Sullivan, Trigg, Christian, Todd, Logan, Simpson, Warren, Allen, Barron, Monroe, Cumberland, Clinton, Wayne, Whitley, Russell, Buchanan, Dickerson, Letcher, Scott, Dade, Catoosa, Walker, Whitfield, Murray
Purpose and Objectives: The Government seeks to enter into a Contract with qualified vendors who can provide a full range of Nursing Home services. Services will be acquired through use of an Indefinite Delivery Indefinite Quantity (IDIQ) Contract.
NAICS code: 623110 and size standard is $27.5 million.
NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/.

Respondent is required to have an active registration in the System for Award Management (SAM) at the time of proposal submission (Prior to Nursing Home Facility inspection date).

Anticipated period of performance: Any supplies and services to be furnished under this contract shall be ordered by issuance of Individual Authorizations by authorized VA Representative. Such orders may be issued from date of award for a 5-year period.
Other important considerations: Respondents must have current state license, and insurance information, and must not have failed a State Nursing Home Inspection within the recent past. Further, respondent will require an inspection by TVHS or Mt. Home personnel.
Information sought: Response to this announcement should include the following information: (1) Completed application form VHA 10-1170 (see attached); (2) Completed additional information sheet (see attached); (3) Copy of State License and Insurance Declaration page; and (4) Copy of active SAM registration (if available).
