i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) This solicitation, 2219-SS32 is issued as a request for quotation (RFQ) in accordance with FAR Part 13.
(iii) The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) can be accessed on the Internet at http://farsite.hill.af.mil/ or at https://www.acquisition.gov/.
(iv) This RFQ is unrestricted and posted as small business set aside. The NAICS code is 115113.
(v) This requirement is the United States Department of Agriculture (USDA), National Animal Disease Center (NADC) to purchase the following item in the following quantities:

* Price shall include all costs needed to accomplish the work described below including incidental materials.
(vi) See the Statement of Work (SOW) in the attached addendum for a description of services required.
(vii) Services shall be rendered at the USDA facility located at:

(viii) The provision at 52.212-1 Instructions to Offerors - Commercial (Jan 2017) applies to this solicitation.
(ix) Evaluation of Best Value will be made in accordance with FAR Part 13 where price, technical capability, and past performance are the factors. Past performance and Technical Capability are more important than Price.
(x) The provision 52.212-3 Offeror Representations and Certifications - Commercial Items (Jan 2017) applies to this solicitation. The contractor shall be currently registered in SAM at www.sam.gov and, either complete the on-line Offeror Representations and Certifications www.SAM.gov or return a completed copy of the Offeror Representations and Certifications with their quotation.
(xi) The clause at 52.212-4 Contract Terms and Conditions - Commercial Items (Jan 2017) applies to this acquisition.
(xii) The clause at 52.212-5 Contract Terms and Conditions Required To Implement Statutes Or Executive Orders Commercial Items (Jan 2017), applies to this acquisition. See the attached addendum for the applicable FAR clauses identified in FAR 52.212-5 which are checked.
(xiii) Additional contract requirements, provisions, and clauses:

(xiv) The Defense Priorities and Allocations System (DPAS) is not applicable.
(xv) Quotation packets are due no later than June 20, 2019 at 10:00am central time. Quotation packets, along with specifications and any supporting documentation shall be emailed to Sarah Schauf, Contracting Specialist, at sarah.c.schauf@aphis.usda.gov.

DOCUMENTS TO BE INCLUDED IN QUOTE: In order to be considered for award offerors shall provide:
1) A quote on company or formal letterhead, complete with DUNS number, detailing the item description, cost per ton, and total price.
2) At least 3 to references. (Government references preferred but will accept commercial references). References shall be provided for the items proposed (or similar) and, if possible, include the email address of the reference person to be contacted;
3) Contractors must be registered in SAM to be eligible to submit a quote.
REJECTION OF QUOTATION: Failure to demonstrate compliance will be cause to reject the quote without further discussions. All responsible sources may submit an offer which will be considered.
NOTE: Guidance in this solicitation for actions within CCR and ORCA are now conducted through the Single Award Management system found at www.sam.gov. Vendors MUST have a record showing "Active" to SUBMIT A QUOTE. No quotes will be considered from vendors lacking active SAM records.
