(i) This is a combined synopsis/solicitation for BLADDER SCANNER, BRAND NAME OR EQUAL, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated.
(ii) The solicitation number is 36C24418Q9328/BLADDER SCANNER/460 and is issued as a request for quotation (RFQ). In the subject line when returning the quote please use the full solicitation number.
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95

Statement of Work: Bladder Scan

This request seeks a solution to a better bladder scan machine that could help with clinical decision and care management. The bladder scan shall help in the determination of bladder function and fluid status. It is through a good bladder scan that a provider could detect that functioning nature of the bladder, the muscle and the nerves that help in the urine formation and utilization and excretion.

This bladder scan machine will make the current work less burdensome to the nurses and makes provider decisions less problematic. It will help with data quality as well. As a patient condition and urinary function can change in a matter of hours, it is critical to have good bladder scan that provide all necessary information and be able to adjust care management plan.

The primary user of this machine will be clinical nurses, providers and other allied healthcare workers that need to know the kidney function. These are people that will be able to analyze every data that the scan is providing adequately.

Main reasons for this bladder scan are:
Appropriate response to the fluctuation in renal function of the patient
Clinical decision that is based on quality data
Collection of bladder function data to prepare clinical staff and providers in their clinical decision making
Determine accurate residual urine and proper identification of the bladder segment/s of the bladder with potential problems

To be able to do all these functions, an automated bladder scan machine will be required. Easiest device that is easily adaptable to the changing patient renal needs. The machine should be the best in class image quality and there should be no software updates charges.
Vendor will provide on-site training to super users on general functionality, data interpretation and how to adjust the screen.
There is a need for a reliable and dependable bladder scan to match the changing nature of healthcare delivery system. The clinical staff rely on quality data for decision making which is most likely be possible through a new updated bladder scan machine.
The benefit ranges from urinary tract infection, excellent patient satisfaction and manageable workload.

SECURITY REQUIREMENTS
Contractor will be required to comply with physical security guidelines if entering the building by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police.
If the Contractor will potentially have access to Veterans in the clinic, then the Contractor must be escorted by appropriate VA personnel at all times while performing work in sensitive areas. If this install cannot take place during non-clinic hours, the Contractors will need to read and sign the Contractor Rules of Behavior and take the Privacy & HIPAA hard copy training unless the PO has some other training for them.
If swapping out Equipment : Also Contractor needs to know that NO Hard Drives leave the facility, If contractor is removing a device and replacing it with a new one, the hard drive in the old device MUST be removed by IT before the device can be taken off station.

(viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda:

(a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.B No remanufactures or gray market items will be acceptable.

(b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.B All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.

(c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.B Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor toB replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.B

(ix) Evaluation of this requirement will be based on PRICE ONLY.
(x) Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer if has not been completed on SAM.gov.
