(i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued.

(ii) Solicitation number 75H71319Q00015 applies, and is issued as a Request for Quote (RFQ).

(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03.

(iv) This requirement is a total Indian Small Business Economic Enterprise (ISBEE) set-aside and the associated NAICS is 339114 with small business size standard of 750 employees.


Non-ISBEE offerors will not be considered for the award. Quoters shall complete and submit the Indian Health Serivce, Buy Indian Act, Indian Firm Representation Form in order to be considered.

(vi) The purpose of this acquisition is to purchase replace six (6) dental chairs and six (6) delivery units at the Warm Springs Indian Health Service Unit, Warm Springs, OR. This is a brand name requirement. Only manufacture certified original and new equipment should be quoted (See attached Statement of Work for details).


(vii) Delivery and acceptance terms for this order is FOB Destination. Delivery Addresses: Warm Springs Health and Wellness Center, 1270 Kot-Num Road, Warm Springs, OR 97761.


(viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Oct 2018).
(ix) The provision 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following factors is price and past performance. The basis of award is the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made to the responsible offeror pursuant to FAR subpart 9.1-To be considered for the award, the offeror shall provide a reseller/partnership certification that evidences the company's eligibility and capability to provide the requested Cisco equipment. Furthermore, the offeror shall conform to the following requirements in the quotation: only manufacture certified original and new equipment shall be quoted; no grey market or refurbished equipment shall be quoted; all dental chairs and units must be vendor certified original and new; no third party or compatible dental chairs or units shall be quoted; and Only A-DEC provided equipment is to be quoted.


(x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer.


(xi) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (Oct 2018) applies to this acquisition. The following addendum applies: Email Addresses for submission of invoices are To be determined (TBD) upon award, Contracting Officer's Representatives (COR'S), and PAO Financial Division.
