Dental operatory rooms require the equipment below according to the Department of Veterans Affairs PG-18-9: Space Planning Criteria Veterans Health Administration August 14, 2014 Washington, DC 20420 Revised: October 01, 2015

Warranty: warrants of equipment for 3 year and 1 year cabinetry.

Installation: Vendor

Note: Delivery of equipment will be by moving van, blanket wrapped, pre-assembled

Justification: New building activation. Equipment listed is required to complete 33 Dental Operatory Suites. Rooms are designed to meet the needs of dental care and ergonomics in the work setting.

Std seamless upholstery - 3625-822 Double Articulating Headrest (NuSimplicity style). Color to be determined

Features: QUAD-FUNCTION FOOT CONTROL, POWER TAPERED
BACK REST, INDEPENDENT MOVEMENT OF TOE SECTION, and VERTICAL LIFT BASE WITH 16" STROKE, POWER TILT FOR TRENDELENBURG POSITIONING.

"    VA will own this equipment.
Electrical Requirements: (STANDARD U.S.)

o    (see manual attached for details)

"    Installation will be required by Bio-Med? Yes
"    Who performs servicing on this machine?
o    There are limited warranties included; this would be sent to Bio-Med for any repairs.
PHI/ISO:
"    It does not have the ability to enter or save PHI.
"    It does not have the ability to connect to the VA Network.
o    This device cannot interface with the VA Network.

"    Contracting Officials will not have access to VA System or VA sensitive information.
"    The C&A requirements do not apply, and therefore a Security Accreditation Package is not required per VA 6. Handbook 6500.6 Appendix A as this acquisition does not require services that involve connection of one or more contractor-owned IT devices (such as a laptop computer or remote connection from a contractor system) to a VA internal trusted (i.e., non-public) network.

Cavicideb" is recommended for disinfecting all other surfaces of the J/V-Generation Chair. The use of any disinfectant, other than Cavicide, may cause premature staining, discoloration or damage to the J/V-Generation Chair's materials.

o    Power Supply: Primary: 100-240V AC, 50/60 Hz.
o    Secondary: 12V DC
o    Operating Amperage: .4 max. - .16 min. Amp
o    Make sure the appropriate electrical line (100-240 VAC, 50/60 Hz) is properly supplied to the power supply housing.

Cleaning & Maintenance:
1. To clean all surfaces, use a typical ALCOHOL-FREE, commercial cleaner or a solution of neutral liquid detergent and water.

2. After washing, a disinfectant may be used following the manufacturer's instructions.
NOTE: Daily cleaning with soap and water or a neutralizer
for iodophors may reduce discoloration.

3. The following disinfectants are acceptable to use:
o Caviwipes
o Banicide
o Coecide

Avoid spraying anything directly on the light. Spray
cloth first, then wipe.
