The Centers for Disease Control and Prevention (CDC) and the National Center for Health Statistics, Division of Health Care Statistics, Hyattsville, MD intends to award a sole source firm fixed price purchase order to the National Adult Day Services Association, 11350 Random Hills Road Suite 800, Fairfax, VA 22030-6044 for the 2020 Adult Day Services Universe Data File.
Statement of Work
TITLE: 2020 Adult Day Services Universe Data File Purchase from the National Adult Day Services Association
I. BACKGROUND

The Division of Health Care Statistics (DHCS) at the National Center for Health Statistics (NCHS) needs a data file that contains the names and addresses of all adult day services (ADS) centers operating in the United States (50 states and the District of Columbia). The data file of the universe of individual ADS centers, containing a set of specific variables for each individual ADS center, is needed to derive the sampling frame for the ADS center sampling stratum of the National Study of Long-Term Care Providers (NSLTCP). The universe lists of ADS centers were used to derive a census of ADS centers for inclusion during the 2012, 2014, and 2016 waves (~5,000), and also to derive a sample for inclusion in the 2018 wave. The 2020 version of the universe list will be used to identify the universe of ADS centers (~5,000) for inclusion in the fifth fielding of NSLTCP ADS survey planned to start in May 2020.
II. PURPOSE
The purpose of the work performed by the National Adult Day Services Association (NADSA) will be to provide a data file containing the most current, complete, and accurate list of ADS centers in the United States.
III. SCOPE OF WORK
NADSA shall provide NCHS with a universe data file containing the most current, complete, and accurate listing of all individual ADS centers in the United States. NADSA updates its universe of ADS centers continuously throughout each year. The universe data file that NADSA sells to NCHS will be used to field a census of ADS centers for the adult day services center survey component of the 2020 NSLTCP; NSLTCP survey data will be collected under the Confidential Information Protection and Statistical Efficiency Act (CIPSEA). The universe data file provided by NADSA shall represent the most current listing of all ADS centers in the United States (50 States and the District of Columbia), and shall contain the NADSA-generated provider identification numbers, names, and addresses for each ADS center, in addition to all of the variables listed in Attachment A. "Data Dictionary" of the Appendix.
IV. TASKS TO BE PERFORMED
A. NADSA will deliver (1) the latest version available of the universe data file of ADS centers in the United States; along with an (2) updated data dictionary for the universe data file (see attachment A. "Data Dictionary"); (3) a detailed draft and final universe data file construction report; (4) an updated state and national contacts list including the names, points of contact, and other information about the organizations that provided a listing of ADS centers (see Attachment B. "State and National Contacts"); and (5) any related documentation, including acquired lists, contact information, and notes developed during the contract period.
B. NADSA shall maintain communications with NCHS throughout the contract period in accordance with the communications matrix outlined in the attachments (see Attachment C. "Communications Matrix").
C. NADSA shall perform these tasks pursuant to the recommended protocol provided by NCHS (see Attachment D. "NCHS Recommended Protocol"). NADSA shall use the recommended protocol as a guide, but shall update or change this protocol as needed throughout the contract period.
V. GOVERNMENT FURNISHED MATERIALS
None.
VI. PERIOD OF PERFORMANCE
The period of performance is the date of award through 2/28/2020.
VII. DELIVERABLES AND PAYMENT SHEDULE
The NADSA data file of ADS centers will be given to NCHS as a Microsoft Excel flat, comma-delimited ASCII file. NADSA will also provide the layout of the files (e.g., variables). All frame-related deliverables for this contract will be delivered to NCHS via the Secure Access Management Services (SAMS) system.
NADSA shall provide the following deliverables:
1. A source file (i.e. the current NADSA Database) that will be used as the foundation for the creation of the Masterfile file;
2. A Masterfile that will be used as the basis of the universe data file and accompanying data dictionary;
3. Draft of "Attachment B. State and National Contacts" with information for tier 1 updated and procured lists from these organizations;
4. Draft of "Attachment B. State and National Contacts" with information for national organizations updated and procured lists from these organizations;
5. Draft 1 of the frame methodology report for completion of stages 1-2 (see Attachment D. "NCHS Recommended Protocol" for a description of stages);
6. Draft of Attachment B. "State and National Contacts" with information for tiers 1-6 and all national organizations updated;
7. Data file with tier 1 cases that have been validated using all lists obtained from tier 1 states;
8. Data file with tier 2 cases that have been validated using all lists obtained from tier 2 states;
9. Data file with cases from tiers 1-6 that have been validated using state lists obtained from tiers 1-6 states and lists from national organizations;
10. Data file with cases from tiers 1-6 that were validated using manual outreach;
11. Final frame methodology report for completion of stages 1-4 (see Attachment D. NCHS Recommended Protocol);
12. Draft and complete universe data files and final data dictionary for the final universe data file (see Attachment A. "Data Dictionary");
13. The agency/organization names and contact information for the state regulatory agencies, state funding agencies, state associations or organizations, national accreditation or certification organizations, and national ADS-affiliated provider organizations or associations that were used to obtain current lists of ADS centers used to create the universe data file (see "Attachment B. State and National Contacts"; all acquired lists of ADS centers obtained through this scope of work.


These deliverables will be provided pursuant to the schedule of deliverables outlined in Attachment E. "Schedule of Deliverables".
VII. MINIMUM VENDOR QUALIFICATIONS
This contract meets applicable policies and procedures in OMB Circular A-120. Contractor shall perform only those services described in the scope of work. All materials prepared for this contract are the property of the government. The contractor shall not release any information related to the work performed or the results obtained without the written permission of the Project Officer.
This contract action is for a services for which the government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b) (SAP) and 41 U.S.C 3304(a) (1). Interested persons may identify their interest and capability to respond to the requirement or submit proposals.
The notice of intent is not a request for competitive quotations; however, all quotations/responses received within (15) days of the issuance of this notice will be considered by the Government. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
