VI.3) ADDITIONAL INFORMATION
The Patient Safety Context.
Research undertaken in the UK and across the world indicates that patients are inadvertently harmed by the care they receive due to preventable incidents.
The National Patient Safety Agency (NPSA) was established as a Special Health Authority in 2001 following the publication of Organisation with a Memory (2000) which highlighted the need to improve patient safety through a reduction in the number of adverse incidents associated with the delivery of patient care.
Patient Safety is a division of the NPSA. The division operates the national system for collecting and analysing reports of patient safety incidents within England and Wales, known as the National Reporting and Learning System (NRLS). A patient safety incident is any unintended or unexpected event that could have or did lead to harm for a patient receiving NHS healthcare (reference Seven Steps to Patient Safety). The divisions overarching role is to drive, implement and support a programme of change across the NHS which leads to a reduction in harm associated with the delivery of patient care and a consequent improvement in the safety of care provided including a patient safety culture.
Patient Safety undertakes a number of data analysis activities to enable the NHS to understand what, how and why patient safety incidents happen, learn from these experiences and take action to prevent future harm to patients. A number of channels are currently used for disseminating analysis, alerts and guidance, and for supporting and collaborating with the NHS to improve patient safety.
The Requirements.
NPSA is seeking to engage with technology Suppliers to provide services to build and operate the technology components that will supplement and add value to the existing National Reporting and Learning System. NPSA is seeking Suppliers to deliver each of the four Lots of system components, support these components, and in some cases, to host the components for the duration of the contract. NPSA is requesting the Suppliers to put forward an appropriate proposal to meet these requirements for one or more of the identified Lots.
The NPSA is aiming to have the new system in place by March 2012, with a key delivery milestone at March 2011 at which point key components must be delivered and operational. These key components will be identified in the ITT documentation.
Lot 1A Interface Engine.
Lot 1B Risk & Incident Management System.
Lot 2 Data Cleansing.
Lot 3 Clustering.
Lot 4 Portal and Information Hub.
NPSA is expecting Suppliers to respond with proposals for earlier delivery of Lots where possible. The Suppliers will lead in the technical design, build and test phase for each Lot and will implement the solution. Post delivery, NPSA is looking to work with the selected Suppliers over a three to five year contract period to host and maintain the solution and make enhancements.
Patient Safety Direct is responsible for delivering in three key areas of functionality:
Capture Data.
Patient Safety Direct will enable the NRLS to capture patient safety incidents and Significant Event Audits, cleanse and share this information for further analysis by NPSA, and other agencies as appropriate.
A customised and simple incident reporting system will be available online for users to report directly to the NRLS. The system will also enable batch and near real time uploads of incident data from Local Risk Management Systems (LRMS) and other stakeholder organisations. The current functionality of the Department of Healths Strategic Executive Information System (STEIS) for the capture and investigation of serious incidents will also be replaced.
Patient Safety Incident data may be cleansed before being made accessible for analysis (depending on the type of patient safety incident and who will be using the data for analysis). This will involve removing duplicate records and identifying and deleting Person Identifiable Information (PII). Once captured there will be the capability to fully manage a patient safety incident, capturing local investigations including Root Cause Analysis outputs as well as NPSA analysis. There will also be improved integration with Third Parties (e.g. MHRA, CQC) through near real-time interfaces for registered users to share relevant patient safety incident data.
Analyse Data.
Patient Safety Incident data and other patient safety information will be analysed in order to identify underlying risks and determine areas of focus for the development of resources and learning to reduce patient harm.
Patient Safety Direct will enable the NPSA to deliver an improved understanding of the causes and contributory factors related to patient safety incidents and the related safety barriers and controls. Patient Safety Incidents will be grouped by these causes, contributory factors and controls to provide a structured view of the main causal factors (or risk areas) within the health service that are leading to patient harm. A Risk Framework will be introduced which will provide a structured hierarchical view of these incident groupings and how they interrelate. The Risk Framework will also contain data from other sources (e.g. Hospital Episode Statistics, Coroners reports, NHS Litigation Authority, etc) in order to provide the most complete picture of risks leading to patient harm.
This structured view of the main causes of patient safety incidents and related barriers and controls, will enable NPSA to build on current processes, focusing analysis to produce the right mitigating solutions, interventions, resources and learning to reduce the occurrence of these patient safety incidents in the future.
The use of clustering tools will support faster and more automated assessment of causes to develop the Risk Framework. User-friendly reporting and analysis tools will enable access to this information according to individual permissions for both NPSA and external users, and result in quicker and more efficient analysis.
Deliver Insight.
The delivery of tailored resources and interactive tools that facilitate greater collaboration between healthcare practitioners and agencies at both a national and local level, thereby reducing patient harm, will be a critical outcome of the Patient Safety Direct programme. Patient Safety Direct will deliver a one-stop-shop for patient safety tools, information and knowledge, enabling users to search for relevant resources across trusted partner websites, share their own stories and learn from others.
It will facilitate greater collaboration ensuring that lessons learned from patient safety incidents at a local level are shared nationally and vice versa. This will be enabled through interactive forums and workgroups and will help to build a stronger culture around improved patient safety.
Patient Safety Direct will provide services and tools that focus upon mitigating the causes of patient safety incidents. These will be created to meet the needs and demands of healthcare professionals, patients and members of the public delivering a personalised and flexible service.
NPSA is looking for Suppliers that are able to demonstrate a strong track record of delivery of solutions required for each of the designated Lots, can bring innovative thinking and solutions, and are able to demonstrate their ability to work with NPSA long term and operate as a trusted partner.
This ITT does not include within its scope the provision of all system components of the NRLS, with outstanding components being delivered by NPSA or existing technology partners. New systems will have to integrate with existing technology.
