This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is market research to: make appropriate acquisition decisions by securing knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses; as well as: 8(a), HubZone and other Small and Large Businesses interested and capable of providing the product(s) and/or services described below. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business; and (i) must provide a capability statement that addresses the organizations qualifications and the ability to provide the product(s) and/or services described below. This procurement is for new Equipment ONLY; no remanufactured or "gray market" items. All Equipment must be covered by the manufacturer's warranty.
The New Mexico VA Health Care System (NMVAHCS), located at 1501 San Pedro Dr SE, Albuquerque, NM, 87108-5128, is seeking potential qualified contractors who can provide: Pentax products, with trade ins, and installation and training, described below. All equipment will be shipped directly to: Phoenix VA Healthcare System, 650 Indian School Rd, Phoenix, AZ, 85012-1892; installation and training will be required after shipment of equipment. Any prospective vendor must be authorized/certified to sell the products, and accept trade-ins as indicated, and install, and train, as described and show proof of that authorization at the time of responding to this Sources Sought announcement.
Important: The Government is not obligated to, nor will it pay for or reimburse, any costs associated with responding to this sources sought announcement request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to the information contained in this announcement, in the event a solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Businesses. However, if responses from Service Disabled Veteran Owned Small Business firms prove inadequate, an alternate set-aside, or, full and open competition, may be considered. No subcontracting opportunities for this procurement are anticipated. The North American Classification System (NAICS) code for this acquisition is 339112, Surgical and Medical Instrument Manufacturing (1,000 employees).
Notice to potential vendors: All vendors who provide products or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. SDVOSB s must be certified in the VIP database at the time a quote is submitted in response to a solicitation, and at time of award, for a quote to be considered.

Please provide the following information clearly and precisely.
1.0 Description of what the equipment being requested is:
Portable recordable Endoscopy equipment used to evaluate functional movements of the laryngeal pharyngeal areas in relation to swallowing assists in the planning and treatment of swallowing disorders. Important parts of the equipment are the Central Processing Unit, slimmer more functional cart, Light source, High-Definition Camera. Flexible and Rigid Endoscopes have already been purchased.

1.1 Description of how the equipment being requested operates:
The HD Endoscopic System is a stand-alone unit. A scope is attached at one end to a camera. The camera is attached to the Central Processing Unit which processes and records the laryngeal and pharyngeal areas of the throat for further review. The resulting photos can be printed out for patient records and for providers to review.
The digital exams are stored on the computer s hard drives where the exams can be reviewed with the patient and/or saved for later reference. The computer systems (i.e. hard drives) will be networked thru the EndoPortal System Server.

Overview
EndoPortal (Server) is an innovative network solution that allows for remote viewing of video endoscopic exams. Once an exam is saved, it is immediately viewable from an office or consultation room, providing clinicians greater flexibility and ensuring most efficient use of workstations. EndoPortal consists of custom server application software and server hardware that connects to your facility existing Local Area Network (LAN), and any additional PENTAX work stations. EndoPortal is a cost-effective complement to the latest PENTAX Medical Digital Recording Module, Model 9310HD.

The EndoPortal Server physical requirements:
Space for 2U server
The server has redundant power supplies so it requires 2x 110V sources from UPS
It has 2 network ports that are set-up in a team configuration so it requires 2x network connections (1Gb each preferred). Static ip preferred.
Should be added to the domain
We prefer a common name be assigned to the server
Utilizes an SSL certificate which can be self-signed or by certificate authority.
TCP Ports 443, 1801 and UDP Ports 3527

9310HD network requirements:
Available, accessible, and active network jack in the exam room.
9310HD can be added to domain or not.
Does not require static ip but one can be assigned.
We can provide MAC address if needed
100Mbps connect minimum with 1Gbps preferred.

1.2 Where will the equipment being requested be used:
In the Audiology and Speech Pathology clinic.

1.3 Who will be using the equipment being requested:
All Speech Pathologists who are properly trained on the equipment.


2.0 Tasks: The Contractor shall perform the following tasks and provide all personnel, supplies, and parts to complete this effort.
Installation of new system
Removal of old system (Trade-In)

Note: The EndoPortal Rack Configuration System will be installed, uploaded, networked, and configured by the VA s Biomedical and OI&T Departments, to be done in a secure and protected location within the facility.

2.1 The period of performance will be 60 days after receipt of award.
NOTE: Following receipt of shipped equipment (4-6 weeks delivery time), VA will contact contractor (Pentax Medical, Eric D Howell | ENT & Speech Product Specialist | Southwest Region) for a convenient date and time for installation and training to take place. Contractor can perform these tasks during normal duty hours, which are from 7:00am to 4:00pm. Contractor will be escorted by VA Employees at all times.
Included in this equipment package request is a Business Associate Agreement (BAA) between The Department of Veterans Affairs Veterans Health Administration and Pentax Medical.
