COMBINED SYNOPSIS SOLICITATION

i) This is a combined synopsis/solicitation for Brand Name Only Zeiss Callisto Eye System for the Lebanon VA Medical Center, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated.
(ii) The solicitation number is 36C24419Q1076 and is issued as a request for quotation (RFQ).
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 20019-02, effective date May 6, 2019, and the Veterans Affairs Acquisition Regulations (VAAR). (iv) This solicitation is set aside 100% for Small Businesses and the associated NAICS 339112 code has a small business size standard of 1000.

Statement of Need

Background:
The Lebanon VA Medical Center (LVAMC) has a requirement for one (1) Zeiss Callisto Eye System. This system will provide computer assistance to Cataract Surgery, enhancing the procedure by allowing precise marking and positioning of the Toric Implants when performing Cataract surgery.

Scope of Work:
The Callisto system provides marker less alignment and exact positioning of the toric lenses along the axis of the patient. This device also assists with creating precise incisions where and the size intended. This device also integrates perfectly with the Zeiss OPMI Lumera used to perform such surgeries and provides a much more ergonomic and comfortable positioning for surgeons and OR staff. The Callisto also provides video recording that provides excellent quality management, as well as a presentation used in teaching. Since Ophthalmology is a Residency program at this facility, this is very important.

Additional Information:
Delivery shall be within thirty (30) days from the time of award. The Callisto and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment in to the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through the OR Nurse Manager, and/or Contracting Officer Representative.

The government will furnish no property to the contractor.

(viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda:

52.214-21 Descriptive Literature (APR 2002)


(a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.B No remanufactures or gray market items will be acceptable.

(b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.B All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.

(c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.B Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor toB replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.B
(ix) Evaluation of this requirement will be based on PRICE and TECHNICAL ACCEPTABILITY
