Sources Sought Notice for Wheelchair Transport Service at Cincinnati VAMC
The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB and VOSB verified vendors who hold a current state of Ohio Emergency Medical, Fire and Transportation Services (EMFTS) certificate to perform wheelchair/ ambulette transportation service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation summarized as follows:
Short Title: Wheelchair/Ambulette Transportation Service for Cincinnati VAMC
NAICS-485991: Special Needs Transportation
Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition
Place of Performance: Department of Veterans Affairs | Cincinnati VA Medical Center (VAMC), 3200 Vine Street, Cincinnati, OH 45220
Anticipated Periods of Performance: 1-Sep-19 to 29-Feb-24 (Base 6-Mnths plus Four Option Years)
General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as specified in the attached PWS. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS.
Anticipated Requirements: The estimated annual not-to-exceed (NTE) quantities for the requirement specified in the attached PWS are summarized as follows:
Wheelchair Transport BASE RATE | One-Way Trips WITHIN 20-mile radius:
5,525 = Estimated one-way trips per year
Wheelchair Transport MILEAGE RATE | One-Way Trips OUTSIDE 20-mile radius:
29,580 = Estimated miles per year
Wheelchair Transport WAIT TIME | 15-minute Increments IAW PWS:
60 = Estimated 15-minute wait-time increments per year

Draft PWS: A draft version of the Performance Work Statement (PWS).
Draft Evaluation Criteria: A draft version of the evaluation criteria for all offers.

Request all small business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code, small business socio-economic size status and a signed copy of their current state of Ohio EMFTS certificate in Adobe PDF format (NTE 5-MB) to perform wheelchair/ambulette transport services in the state of Ohio for the subject anticipated requirement to bradley.burhite@va.gov not later than 27 May 2019. Only those prospective small business prospective offerors who are certified under NAICS 485991 and submit a current signed copy of their Ohio EMFTS certification in response to this sources sought notice will be considered responsive for the purposes of this FBO posting.
