This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA258-14-Q-0604 and is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-74 effective 05/30/14. The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 with a small business size standard of 500 employees. This requirement is: set aside for service disabled veteran owned small business (SDVOSB).
FOB Destination shall be Albuquerque, NM 87108-5154.

20 ea Staxi ST010-L Transport/medical chair blue; 20 ea Staxi ST059 cushion black; 20 ea Staxi ST075 oxygen tank holder; 22 ea Staxi ST084 telescoping IV/IVAC Combo Pole; 2 ea Staxi ST310-L bariatric chair blue- includes cushions; 2 ea Staxi ST-075-B oxygen tank holder- includes bracket kit. -CHAIRS DESCRIBED IN THE SOLICITATION AT: STATEMENT OF WORK. PLEASE REFER TO THE SOLICITATION for ALL LINE ITEMS.
