VoIP Telephone Service for Lockport Service Unit
The Indian Health Service, Nashville Area Office is proposing to locate a vendor to provide communications services for the Lockport Service Unit located at: 6507 Wheeler Road, Lockport, NY 14094-9416.
We would like to provide your company the opportunity to offer a proposal to install and provide basic communications services (VOIP and equipment purchase) to our service unit. If you would like to submit a proposal on the work, please construct an offer for the compensation related to your services as defined in the following request for proposal and complete all of the documentation included in the package. The standard form (SF), SF-1449-12 must be completed along with any other signature blocks for consideration.
