Vision Insurance
Agency is seeking proposals from qualified firms to provide a fully insured voluntary vision plan for eligible employees and their dependents. Agency does not offer a vision plan currently. The effective date of the new plan will be January 1, 2011, with the first renewal January 1, 2012. Rate guarantee through December 31, 2013 requested.
The effective date will be January 1, 2011, for an initial contract period of one (1) base year, and two (2) one-year option years, to be exercised at Agency's discretion.
