Vision Insurance
The State is issuing this Request for Proposal, for
the purpose of selecting a qualified contractor to provide Vision Insurance.
A contract resulting from this Request for Proposal will be issued for a period of three (3) years
effective July 1, 2010 through June 30, 2013, with the option to renew for two (2) additional two (2)
year periods as mutually agreed upon by all parties.
