Network Contracting Office (NCO) 9 intends to release solicitation number 36C24918Q9477. NCO 9 intends to enter into a firm-fixed price, indefinite delivery requirements contract (ID/R) for prescription eyeglasses on behalf of Veterans Integrated Service Network (VISN) 9 VA Healthcare Systems (Medical Centers and Outpatient Clinics). The contract will be composed of one (1) base period and four (4) one-year option periods, for a potential total contact period of five (5) years.
The North American Industry Classification System (NAICS) code for this requirement is 339115 Ophthalmic Goods Manufacturing with size standard of 1,000 employees. The product service code (PSC) is 6540 Ophthalmic Instruments, Equipment and Supplies. It is the intent of NCO 9 to procure these items from a service-disabled small business (SDVOSB) reseller.
The contractor shall provide prescription eyeglasses for the following locations: VAMC Mountain Home, TN; CBOC Knoxville, TN; VAMC Nashville, TN; VAMC Murfreesboro, TN; CBOC Chattanooga, TN; CBOC Clarksville TN; VAMC Memphis TN; CBOC Nonconnah, TN; CBOC Jackson, TN; VAMC Louisville, KY; VAMC Lexington, KY; and CBOC Somerset, KY.
The contractor shall provide prescription eyeglasses to eligible veterans. All Lenses shall be fabricated and provided in accordance with the standards set forth in the current edition of American National Standards for Prescription Ophthalmic Lenses - Recommendation ANSI Z80.1 or latest edition. All ophthalmic lenses shall be in accordance with Food and Drug Administration (FDA) Regulation (2) CFR, Parts 3 and 4 for impact resistance, unless otherwise specified in the order.
All frames shall be provided in accordance with the current edition of the American National Standard Requirements for Dress Ophthalmic Frames ANSI Z80.5 or latest edition.
The contractor shall supply a licensed Dispensing Optician/On-Site Fitter to each VA Medical Center/facility in VISN 9, five days a week, Monday through Friday, 40 hours a week, excluding federal holidays. If a State does not require an American Board Certified Dispensing Optician, the Dispensing Optician must abide by the State Licensing Requirements. Proof of State licensing requirements must be provided.
Interested parties shall be register in the System for Award Management (SAM) as prescribed in FAR Clause 52.232-33. The SAM registry can be obtained by accessing the internet at www.sam.gov or by calling 1-866-606-8220.
All SDVOSB Offerors must be verified in the Vendor Information Pages (VIP) database at https://www.vip.vetbiz.gov/.
It is anticipated that the full solicitation will be released on FedBizOpps.gov at www.fbo.gov on or about July 31, 2018.
