Subject: VA261-17-Q-0499 Request for Quote (RFQ) for 4.0 FTE Board Certified, Diagnostic Medical Sonographer (Ultrasound) with subspecialty in Abdomen, Pelvis, and small parts.

DUE: 4 MAY 2017, 2:00 p.m. PST

Description: VA Palo Alto Health Care System (VAPAHCS) seeking quotes for 4.0 FTE contractor staff that are board certified, Diagnostic Medical Sonographer (Ultrasound) with subspecialty in Abdomen, Pelvis, and small parts. This is a firm-fixed price order, for a period not to exceed 52 weeks, with various shift differentials (See Section 3 of PWS).

Location: VA Palo Alto Health Care System (VAPAHCS), Radiology Department, 3801 Miranda Ave, Palo Alto CA 94304.

Period of Service: 52 weeks. NOTE**Due to security requirements, the average time from contract award to actual first day of performance is 10 weeks. Please ensure your candidates are aware of this before initiating the background and credentialing process. VSC clearance is a requirement; performance shall only begin after VSC has granted permission to onboard.

Mandatory- SAM Registration- (http://sam.gov) before award. Note, the Government NEVER charges prospective contractors to register in SAM. If a payment is requested from a website, it is not the official government site.

VIP (Vendor Information Pages Database) ONLY IF SDVOSBs or VOSBs
If Contractor is self-certified as either Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) or Veteran-Owned Small Businesses (VOSBs), you must be registered in VIP http://www.VetBiz.gov.

Evaluation procedures: LPTA including Past Performance. Quotes will only be reviewed for price after evaluated as technically acceptable and a satisfactory past performance rating. Provide no more than (5) five resumes. Candidates that are currently cleared to onboard or held clearance approval in the past 180 days are highly encouraged to submit their quote.

Pricing Instructions:
Quotes shall be filled out using RFQ CLIN pricing schedule each (Max 6 pages. 1 page for each candidate, up to 5 and the Summary page). All costs shall be broken out e.g. shift differential, on call, pager, call back, per diem, travel, etc. If you are a GSA vendor and offer discounts to the VA, please indicate the FSS Schedule rate compared to the discounted rate. Quotes without itemized pricing may not be evaluated for award. This table can be edited to reflect contractor costs as needed for quote.
