VA LONG BEACH / VA LOMA LINDA / VA GREATER LOS ANGELES BI-ANNUAL INSPECTION, CERTIFICATION, TESTING OF FUME HOODS, VENTILATORS, BSC, ISOLATORS, AND PHARMACY CLEAN ROOMS
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 - streamlined procedures for commercial items, in conjunction with FAR Part 13 - Simplified Acquisition Procedures, specifically FAR Subpart 13.5 - Test Program for Certain Commercial Items; and as supplemented with additional information included in this notice.
This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-77, dated October 10, 2014.
Solicitation VA262-14-Q-0037 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm Fixed Price award with a base year and up to four (4) option years.
This procurement will be issued under a total small business set-aside. NAICS Code is 541690 and size standard is $15 million.
The Government is seeking a Bi-annual Inspection, Certification, and Testing of Government-Owned Fume Hoods and/or Ventilation Devices, Biological Safety Cabinets (BSC), Laminar Flow Devices, Isolators, Biosafety Hazard Hoods, Sterile Workbenches, Radiation Safety Hoods, Buffer and/or Ante spaces, and Pharmacy Clean Rooms.
Inspection, certification, and testing shall be in accordance with the following policies and recognized standards; and any other recognized standards within the industry norm:

Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a Acceptable/Unacceptable evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government.
Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document (unless electronically completed on www.sam.gov ). As an alternative, Offeror(s) may use the Price/Cost Schedule of the attached SF 1449 for quote submission.
