BRIEF SCOPE OF WORK

GENERAL

Location: Jack C. Montgomery; 1011 Honor Heights Drive; Muskogee, OK 74401

Overview: Repair facility illuminated signage at the top of building 53. Building 53 is a five-story healthcare facility. Signage is approximately 150ft from the ground to the location of signage. Please ensure all design or lettering completely illuminates and report back and foreseeable future issues that can be corrected or needs to be corrected in the future.

2. SCOPE

The materials and services performed under this contract shall address the items of work noted below to provide the repair of all illuminated signage at the top of building 53. The tasks to be completed under this project include the following:

Install stage or mechanical lifting device to safely reach and repair signage.
Trouble shoot signage that is currently not illuminated.
Identify damaged or defective materials for replacement.
Identify existing materials that may be need to be replaced in the future.
Order replacement parts for all illuminated signage on the top of building 53.
Install and or repair and replacement parts or materials identified.
Ensure all lighting is properly repaired and illuminated.
Remove stage or mechanical devices.
Replace all light bulbs.
Provide maintenance.

3. DELIVERABLES

All labor, equipment & materials, including bulbs, formwork, rebar, lifting equipment, delivery vehicles to repair illuminated signage. Ensure no VA property is damaged in the process of repairing signage. After troubleshooting system contractor is responsible for providing a report of any foreseeable maintenance issue to the COR.


4. INSPECTION AND ACCEPTANCE

The VA, designated COR will inspect the project work.
Contractor is responsible to meet all local, state and federal regulations.
All VA contractor regulations must be followed at all times.
All contractors must have VA contractor badges in clear view at all times while working on VA property.


5. GOVERNMENT FURNISHED MATERIALS

6. AVAILABILITY

Site is available upon proper 2-week notification for access.

7. CONTRACTOR REQUIREMENTS:

Contractor will provide all labor, equipment and materials required to perform the work identified above.
Meet on site with VA COR and Contracting for an on-site pre-construction meeting
Coordinate all construction activities with the COR prior to beginning work.
Care must be taken to avoid damage to all VA property when performing work.
Any disturbed areas outside the intended scope of work will be restored to pre-existing condition at no cost to the VA.
Remove all equipment and excess materials remaining on site within 5 days of project completion and final inspection.
Work hours are from 7am 4:30pm without prior approval
