Statement of Work:

The work consists of providing under-bridge access equipment, operator, and driver for the use of bridge inspection and maintenance. The work assumes three different types of equipment depending on the weight and reach of the unit. All equipment shall be capable of the following:
• Provide an Under Bridge Inspection Vehicle (UBIV), capable of the following:


• Travel on paved and unpaved road surfaces in remote mountainous areas of the National Forest. Some Forest roads will have steep grades, sharp curves, pot holes and loose gravel.


• Have the ability to operate in, around, through and over all types of obstructions including bridge trusses and girders.


• The system shall be able to be deployed, reaching across sidewalks and over fences to gain access to under bridge areas.


• Be able to set up, deploy and operate the UBIV within an 11' wide lane.


• The inspection truck shall be in good working condition. Any deficiencies or problems noted in the truck's inspection shall have been corrected.


• Provide minimum 117 VAC 4 AMP AC power source to the inspection bucket.


• Provide a communication system and backup communication system between the driver and the operator


• The driver and operator shall both be fully trained and certified to drive and operate the vehicle and crane. A minimum of 2 years' on the job operating experience is required.


• The Government will only pay for services received. The equipment pricing shall include all miscellaneous expenses, parts, fuel, lubricants to keep equipment in good working condition. The Government will not be responsible for payment of downtime due to mechanical failure, the lack of a qualified operator, or any reason within the control of, or due to the fault or neglect of the contractor.


• Provide and have readily available the following basic firefighting hand tools (2) shovels and (1) Pulaski fire ax



SUBMITTALS
1. UBIV truck description with capacity and reach
2. Provide certification that the inspection truck has been inspected in accordance with ANSI 92.8 within one year of when the truck will be used for this contract and all documentation of repairs from said inspection.
3. Provide documentation that Operator and Driver have a minimum of 2 years on the job experience with a UBIV. Aerial devises and other equipment than a UBIV does not qualify as on the job experience.

The work will be located within the following National Forests: Gifford Pinchot, Mt. Hood, Mt. Baker-Snoqualmie, Olympic, Okanagan-Wenatchee, Lolo, Flathead, and Kootenai, Shoshone, Shasta Trinity, Klamath, El Dorado
