This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0327 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 333999. This is a small business set aside. All responsible sources may submit a proposal which shall be considered by the contracting officer.
Requested Items:
The John D. Dingell Medical Centers requires a purchase order to cover replacement of American Ultraviolet lamps. These replacement lamps will go into the 62 air handling units throughout the hospital. The purpose of these lamps is to reduce or eliminate, DNA-based airborne contaminates. The vendor will supply the following:
1. America Ultraviolet OEM lamps.
2. Part Number GCL-48 - Lamps for all CC48 fixtures - (Qty. 260 each)
3. Part Number GCL-60 - Lamps for all CC60 fixtures - (Qty. 4 each)
4. As part of the purchase order these lamps will be shipped to John D. Dingell VA Medical Center, 4646 John R. Street, Detroit, MI 48201
5. Theses lamps will be supplied on-site at DVAMC by no latter then 6/30/2014
