Transition Coordinator Services
This is a Request for Proposal (RFP) for Transition Coordinator Services issued by Department of Health and Social Services, Division of Medicaid & Medical
Assistance (henceforth referred to as The Division).
The primary goal of Transition Coordinator services is to provide intensive supportive services
to individuals interested in transitioning from a qualified institutions (public and private
Medicaid enrolled nursing facility, Intermediate Care Facility for the Mentally Retarded
ICF/MRs - and the Psychiatric Center - DPC) to qualified home and community based
settings with the provision of services and supports that meet those individuals needs and
preferences.
