This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
The Mission and Installation Contracting Command - Fort Sam Houston intends to award a firm fixed price contract on a sole source basis in accordance with FAR 6.302-1 to Four-D Software, Inc., 131 N. First Street, Suite #1, Pulaski, TN 38478-3234 for access/subscription to Four-D's Medatrax data tracking system, a web-based platform for case record management, care plans, and grading required by the US Army Graduate Program in Anesthesia Nursing (USAGPAN). Access/subscription to the Medatrax software technology developed by Four-D Software, Inc. is a web-based Nurse Anesthesia Program Case Record Management automation technology with optional Mobile Device application support for Cases and Evaluations. The USAGPAN consists of one Phase 1 Site located in San Antonio Texas and nine geographically distant Phase 2 clinical sites. The services are commercial within the meaning of FAR Clause 52.212-1. The contract will be issued on 5 June 2014. Estimated cost is $127,525.15. The National Board of Certification and Recertification for Nurse Anesthesia (NBCRNA) has recently transitioned to a totally electronic documentation system. Subsequently, all transcript documents submitted by candidates for the certification exam must be in electronic format. The Medatrax software technology developed by Four-D Software, Inc. is recognized by the NBCRNA as meeting the requirements for electronic record keeping and submission.
