This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
Solicitation number W912D0-14-T-MEDICALGAS is hereby issued by the Regional Contracting Office, Alaska, 1064 Apple St., Fort Wainwright, AK, 99703, as a Request for Quote (RFQ) for Annual Medical Gas Inspection and Certification Services. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-74, Effective 1 July 2014. This acquisition will be solicited using the North American Industry Classification Standards (NAICS) code 541990 size standard is 15 million dollars.
Description of Requirement:
The contractor shall be able to provide materials, labor and equipment to inspect, test, and certify medical gas systems installed in the Bassett Army Community Hospital, building 4076 as identified in the attached document titled quote mark Attachment A - PWS quote mark . The Medical Gas Systems inspection and certification services will be conducted on an annual basis and this contract will include a base year + four 12-month option periods. Contractor shall be certified in accordance with ASSE series 6000. Contractor shall provide a copy of this ASSE certificate to contracting officer prior to award. A Determination of Contractor Responsibility is required to be completed for all purchases. In accordance with the FAR 9.104 part of that process includes Financial Responsibility. All contractors refer to Attachment D-Financial Institution Information. Award shall be made only to contractors who have registered with System for Award Management (SAM). The Government will enter into a contract resulting from this combined synopsis/solicitation to the responsible offeror(s) whose offers conforming to the solicitation will be most advantageous to the Government.

Payments will be made in accordance with FAR 52.212-4(g) the contractor shall submit all invoices electronically utilizing Wide Area Work Flow (WAWF). Vendors are required to have a WAWF account. Procedures for using WAWF are found in DFARSS 252.232-7003, Electronic Submission of Payment Requests.
Quote Information:
Quote shall be structured to show an Item Number breakdown of the requirement and should reflect a firm-fixed-price for each. See Attachment B- QUOTE Sheet. All questions must be submitted by close of business 30 July 2014. Quotes shall be submitted and received no later than 25 August, 2014 at 1500 hours (3:00 P.M.) Alaska Standard Time. Quotes may be mailed, emailed, or hand carried, to both Points of Contact listed below.
