The Department of Veterans Affairs Network Contracting Office 21 is conducting a market survey for qualified firms capable of providing the San Francisco VA Medical Center with all labor and materials needed for a comprehensive temporary shelter accommodation services (residential room or board; meals; laundry; restrooms; transportation; and therapeutic services).
Note: Must be a qualified and certified vendor able to provide these types of services in accordance with all applicable California state laws.
1) Residential Room and Board:
The Contractor shall provide temporary shelter accommodations for 30 eligible male or female Veterans who are beneficiaries of the Veterans Affairs Medical Center, 4150 Clement St., San Francisco, CA 94121 (hereafter known as SF VAMC) in accordance with all terms, conditions, provisions and schedules of this solicitation and resulting contract. Unless specifically included in this contract, the daily rate established shall include the services listed in this document and will also include all services or supplies normally provided other persons by the facilities without extra charge. The Contractor shall be required to provide said services on a 24/7 basis, except in cases of emergency.
2) Food/Meals:
The facility shall provide at least two nutritious meals or their equivalent. These shall be served daily at regular times. Food shall be prepared, served and stored under sanitary conditions. The facility shall establish and maintain sanitary procedures for washing dishes, cleaning equipment and work areas, and disposing of waste.
3) Laundry, Showers, Toilets, Secure Space for belongings:
The facility shall provide weekly bed and bath linen service and cleaning of the shelter space weekly, or more often as needed, for a total of 30 Veterans per day. The communal toilets and showers must be cleaned at least two (2) to three (3) times per day. Laundry facilities shall be provided for residents to do their own laundry. Showers and toilets must be kept clean. A secure space for belongings, examples include dresser or cabinets, shall be provided.
4) Transportation:
The Contractor's facilities must be geographically accessible by public bus line, to the SF VAMC and must be safe, spacious, and well-maintained. The Contractor shall assist the Veterans with local public transportation to scheduled meetings and appointments. The Contractor will be expected to help the Veteran access public transportation, including providing information and instructions necessary to enable Veterans to utilize public transportation.
5) Therapeutic Services:
The Contractor shall provide on-site case management. Case management will be focused on assisting the Veteran with making a viable discharge plan to permanent housing, transitional housing, or residential treatment. Case management will include referral to resources in the VA system and in the community. Services which the contractor must be able to furnish include:
a. Individual Counseling/Clinical Case Management: The Contractor shall provide access to counseling for Veterans with co-occuring disorders including mental health and substance abuse disorders. The Contractor shall discuss with Veterans their legal issues, personal income, expenditures, and savings goals. The Contractor shall assist Veteran in setting up an account or provide other interventions to help Veteran save a significant portion of their income while in the program. The Contractor shall discuss and assist with Veterans their primary and secondary housing plans, including identifying options and assisting with applications when necessary. In addition, the Contractor shall provide other individual counseling as appropriate - such as counseling on self-care skills, adaptive coping skills, and vocational rehabilitation.
b. VA referrals regarding Veterans: Clinical care should also include referral to Veterans Affairs inpatient and outpatient treatment for medical issues, addiction, mental health issues/PTSD, unemployment, and homelessness including occasional transfer to other VA residential treatment programs.
d. Living Skills: Assistance to develop responsible living patterns and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships.
e. Community Involvement: The program will promote community involvement that promotes the Veterans continued healthy engagement in the community discharge.
g. Discharge Planning: The clinical case managers will provide assistance with discharge planning. Housing needs will be assessed upon arrival and resources will be coordinated for discharge to a successful community placement.
The purpose of this notice is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, HubZone, 8(a), small disadvantaged business, small business, or large business). SDVOSB/VOSBs must be registered in www.vip.vetbiz.gov and all firms must be registered in www.sam.gov. Responses to this notice will be used by the Government to make appropriate acquisition decisions.
Responses to this notice should include company name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and the size of your business pursuant to the following questions: (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified HubZone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned or Veteran Owned Small Business?
Responses to this notice must be submitted in writing (email or fax) and received no later than May 5, 2017. No telephone inquiries will be accepted. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
