T-SHIRTS AND POLO SHIRTS

This bid will be awarded separately to the lowest responsive, responsible bidder(s) who submits the
lowest and best total price for all T-Shirts and/or all Polo Shirts along with SAMPLES of select sizes as
indicated by asterisks. Failure to submit pricing for all items and submit samples for selected items will
result in rejection of your bid. T-Shirt and Polo Shirt brands quoted are to be delivered to:
Ann Cook, Buyer, MSD's Main Office Building, 700 West Liberty Street, Louisville, Kentucky, 40203 by the
date and time noted above. (Electronic or faxed quotes will not be accepted).
