Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; are being requested, and a written solicitation document will not be issued.
This solicitation is a "request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97.
The associated North American Industrial Classification System (NAICS) code for this procurement is 332999, with a small business size standard of 750 employees.
VA Hudson Valley Healthcare System Montrose Campus at 2094 Albany Post Road is seeking to purchase Brand Name Only TBJ Incorporated Triple Bowl decontamination work sink for installation and delivery to FDR SPS department.
All interested companies shall provide quotation(s), for the following:

T304 stainless steel construction
Heliarc welded construction
All exposed welds ground and polished to #4 finish
(3) 22 x 18 x 10 B= deep sink bowls with etched gallon indicator markings in each bowl
Sink bowls will be centered leaving approximately 18 of counter space on left side and 18 on the right side
(1) Deck mounted hot/cold water faucet at left sink with detergent line hole and DEMA tip
(1) Deck mounted hot/cold water combination spray rinse / faucet at center sink with detergent line hole and DEMA tip
(1) Deck mounted RO/DI pure water faucet at right sink
(1) Deck mounted Selecta compressed air pistol at right sink

Installation responsibilities
Receiving of new sink
Complete removal of old sink
Complete installation of new sink, including packing materials, plumbing, electrical connections.
All removal and installation to be performed over a weekend.

Services
Equipment delivery, as well as pull out old sink & install new sink required.
The delivery/task order period of performance is 05/31/2018-07/31/2018.
Delivery shall be provided no later than 15 days after receipt of order/award of contract. Delivery terms Destination. The contractor shall deliver line item(s) 1-2 to VA Hudson Valley Healthcare System Montrose Campus.
Place of Performance-Delivery, pull out old sink & Installation of new sink required-Drawings to be approved as well as signed off when finished.

Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. A written authorized buyer letter submission is required.
**Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.B All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions**.B

This is an open-market combined synopsis/solicitation for TBJ Incorporated Triple Bowl decontamination work sink with installation as defined herein.B B The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all bids must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
Quoters shall list exception(s) and rationale for the exception(s).
Submission shall be received not later than 05/30/2018 10 A.M at email address listed above for Contracting Officer. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). ONLY Faxed or e-mailed quotes will be accepted.
