THIS IS A SUBCONTRACTING OPPORTUNITY
The Carl D. Perkins Job Corps Center, operated by Horizons Youth Services for the Department of Labor, is seeking qualified providers for dental hygienist services. The requirement will consist of seven (7) hours per week until March 31, 2016, with the possibility of two (2), six (6) month extensions.
Deadline for proposals will be March 29, 2015, 12:30 pm.
The attachment includes the subcontract, scope of work, general terms and conditions, applicable FAR clause references and evaluation criteria. As part of the submission, offerors should include all licenses, relative past performance, pertinent credentialing, any applicable insurance coverages and the Vendor Proposal Sheet found in the attachment.
