This Request for Proposal (RFP) is for an electronic informed consent software solution that can operate in VA s current architecture environment for the near future. Veterans Affairs (VA) electronic health record changes are projected to move this requirement to a web-based or cloud-based electronic health record architecture environment. VA is currently in the planning phase for the future informed consent software that will follow this procurement action.
In accordance with FAR 12.603 the following information is provided:
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) The solicitation number is VA118A-17-R-0135 and the solicitation is issued as request for proposal (RFP).
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94, 2005-95.
(iv) The solicitation is not set-aside, the associated NAICS code is 511210, and small business size standard is $38.5M.
(v) See Section B below for a list of line item numbers, quantities, and units of measure including options.
(vi) See Section B and Section D below for a description of requirements for the items to be acquired.
(vii) See Section B and Section D for delivery dates and place of performance and delivery. Acceptance is FOB orgin.
(viii) FAR 52.212-1, Instructions to Offerors Commercial, applies to this solicitation and an addenda to the provision is provided below is Section E.
(ix) FAR 52.212-2, Evaluation Commercial Items, applies to this solicitation, see Section E for specific evaluation criteria.
(x) Offerors are advised to completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer.
(xi) FAR 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition and an addenda to the clause is provided in Section C.
(xii) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and additional FAR clauses applicable to the acquisition are identified in Section C.
(xiii) Additional contract requirements, terms, and conditions are identified in Section B, Section C, Section D, and Section E.
(xiv) The Defense Priorities and Allocations System (DPAS) is not applicable.
(xv) All offers are due July 28, 2017, at 3:00:00pm Central Time to diana.armesto@va.gov.
