Sickle Cell Disease and Trait Community Outreach
The State is seeking proposals to identify a contractor who will develop and administer a statewide program to address Sickle Cell Disease (SCD) and Trait in the adult and adolescent population. The purpose of the funds is to identify adults with SCD and Trait, assure linkages to medical homes, provide transition planning and referral to ensure efficacious transition from pediatric to adult services.
