Services Coordination and Resource Development for the Home and Community Based Medicaid Waivers Serving Adults with Disabilities and Adults with Traumatic Brain Injuries
The Request for Proposal is designed to solicit proposals from qualified vendors who will be
responsible for providing services coordination and resource development for adults with
physical disabilities and adults with traumatic brain injuries served through the home and
community based Aged and Disabled and Traumatic Brain Injury Medicaid Waivers at a
competitive and reasonable cost. Proposals that do not conform to the mandatory items as
indicated in the Request for Proposal will not be considered
