1.0 Background

The purpose of this Statement of Work (SOW) is to provide shoes to employee s that have a safety need according to uniform policy. This statement of work is to provide shoes any size, style, and texture for the purpose of work.

2.0 Quality of Boot
Oil Tan Full Grain available in waterproof or non-waterproof, 1.4-2.4 mm thickness
Tumbled Full Grain non-waterproof, available in 1.4-2.4 mm thickness
Distressed Waxy Nubuck non-waterproof, available in 1.8-2.4 mm thickness
Gore-friendly Waterproof meets Gore MVTR standards, available in 1.4-2.4 mm thickness
Embossed Oily Full Grain available in waterproof or non-waterproof, 1.4-2.4 mm thickness
Fire Resistant meets NFPA standards, available in waterproof or non-waterproof, 1.4-2.4 mm thickness
Antique Full Grain & Nubucks available in waterproof or non-waterproof, 1.8-2.4 mm thickness
Camouflage patterns through Jordan Outdoor and Haas Outdoor, available to licensed manufactures/dealers only, 1.0-3.2 mm thickness
3.0 Boot Brands requested but not limited to:

Dr. Martins
Justin Work
John Deere
Nautilus
Rockport
Wolverine
Please provide a listing of shoes available and pricing being offered to the Phoenix VAMC.
4.0 Delivery or Pick up

Employee will go to the vendor no more than 20 miles from the facility on their on time or vendor will to come to Phoenix VA at 650 E. Indian School roads, in Phoenix Arizona 85012 at least twice a month. Employee will be issue 1 pair of shoes per year unless boot/Shoe is worn beyond repair and or unserviceable and presents a safety risk to the employee. To be determined by the employee and the vendor.
Employee will present a voucher or some indicator to vendor for shoes and vendor will document employee s name and other documentation, date boots where issued. Vendor will submit Invoice electronically for payment.

5.0 Period of performance

The period of performance is one (1) year from date of award


6.0 Repair

Vendor will provide repair service on all shoes or replacement whichever is agreed upon by employee and vendor. Each shoe not to exceed $160 per pair per person, any shoe valued above this amount shall be billed to the employee. Vendor will fit and measure each person to the right shoe based on the employee s foot and taste.


7.0 Misc

Amount of contract is estimated at $24,000 a year or 150 Pairs of BOOTS PER YEAR.
Styles: approximately 20 styles requested
Men and Women s styles
Shoes are for the following service Laundry Dept, Housekeeping , Paint shop, Grounds, Food and Nutarian , Engineering and Grounds.(ALL EMS PROFESSIONALS)
