This is not a solicitation or request for quote (RFQ). This is not a request for proposal (RFP). This is sources sought notice to industry and the only information is posted on FBO web page. The attachments included on the FBO page do not include any additional information. If your company is interested please read this notice entirely and submit the required information, as requested herein. VA is seeking current qualified sources who have interest and capability of performing annual preventive maintenance and repair services on an Abbott (St Jude) EnSite Velocity-Cardiac Mapping System at the VA Salt Lake City Health Care System. Current in qualifications means fully certified Abbott (St Jude) certified repairer, who would have immediate access to OEM parts (in the event of a follow-on repair being required). Immediate and direct access to OEM parts means excluding gray market items, excluding remanufactured items, and where vendor shall be an OEM authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services for maintenance, repair or evaluation on repairs needed for medical equipment (i.e. replacement parts). Vendor s confirmation of the latter qualifications can easily be verified by providing, upon request by the VA Contract Specialist or Contracting Officer, an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.B All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. Additional qualifications include recent experience (within past 3 years) of performing services of a comparable size, scope and complexity, as well as current and valid registration in SAM.gov. Please respond to VA Contracting Officer, Monica Gary, at monica.gary@va.gov with company details, to include company size as it relates to NAICS code 811219. In your reply, please provide company DUNS number, company OEM documentation for qualification of access to OEM parts as described above, and state if you would be performing services as a primary contractor or subcontracting the services. Include your company s recent experience, be it a US Federal contracted service or a service performed external to US Federal government. Please reference the FBO Sources Sought Notice number in all communication.
