COMBINED SYNOPSIS/SOLICITATION
CARDIOLOGY INFORMATION AND ARCHIVING SYSTEM
SYRACUSE VA MEDICAL CENTER
This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued.
(ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number VA242-17-Q-0762.
(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95.
(iv) This procurement is being issued as on an UNRESTRICTED basis. It is the Government s intent to issue a firm fixed price, single award as a result of this solicitation. The North American Industry Classification System (NAICS) code is 339113, Surgical Appliance and Supplies Manufacturing, with a small business size standard of 750 employees. This solicitation is UNRESTRICTED.
(v) The Contractor shall provide all parts and labor necessary to deliver a cardiology information and archiving system (CIS) and training in accordance with the attached Statement of Work. The System is to be of same brand/name or equal to the Siemens Syngo Dynamics CIS. The salient characteristics are identified within the Statement of Work. The CIS System must be compatible with existing Siemens equipment already employed at the Syracuse VA Medical Facility.

(vii) EVALUATION: In accordance with FAR 52.212-2. Quotes will be evaluated based upon best value to the Government. Technical acceptability is based upon compatibility with existing equipment and current IT baselines, delivery and installation lead time, and past performance. Pricing will also be evaluated.
Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions

(viii) CONTRACTOR REMITTANCE ADDRESS: All payments by the Government to the contractor will be made in accordance with 52.232-34, Payment by Electronic Funds Transfer Other Than System For Award Management,
(ix) INVOICES: Invoices shall be submitted upon final installation of goods
(x) GOVERNMENT INVOICE ADDRESS: All Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests.

If proposing an equal system, provide at least 2 references showing success of proposed system compatible with Siemens equipment as listed in the SOW.
